If you check the "Yes" box, to have your name placed on the Hall of Fame listing, then whatever name you enter will be listed. If you include your year of birth and/or country of residence, those too will be included in the list. That's the only reason for those fields to exist, just for the fun of it.

The First name, Last name & Email fields are mandatory. After all, what's the point of a certificate with no name? After you've paid the $4 fee, you'll return to a page that will provide you with a download button for your certificate. We also will send you an email, to the address you've entered on this page. That email will include your personal, unique link to this site that will recreate this certificate and offer you the option to download it again if you need to do so.
